I need your great help in correcting my grammar as well as where I should exactly be sending my nomination letter.
I am DT, a Physical Therapist in Canada. I had car accident of January 16, 2014. I was brought to Lakeridge Hospital and diagnosed me with concussion. Based from my experiences some of the family doctors weren’t have in depth knowledge about the complexity and impact of car accident in the brain. They advised me to go back to work as Physical Therapist because there was nothing wrong with me. I was hopeless cause I was experiencing double vision, vomiting, pressure headaches, general body and mentally fatigue, poor concentration and coordination and later on I developed seizures.
After 1 and half years during the course of my ordeal I came to know this Case Manager. He was the only one who believed in me when no one believes. He stood by me. Just mere glanced he knew that there’s something wrong with me.
He has made a measurable impact over the past 5 years as a passionate Case Manager.
His activities are as valued as his skills and they include; working with me as a client to help me to understand and overcome my fear. He’s my advocator for ensuring the best interest of me. He was both my eyes, ears and mouth.
He ensured that the services outlined the clinical pathways were provided. He coordinated the clinical multidisciplinary team . He oversee the evaluation and satisfaction of my needs, seeks to ensure the continuity of my care by avoiding fragmentation and duplication of my services. He invites members to attend meetings and maintain relations among consultants and involved in the follow up.
He’s like a big brother to me who is most in need of one. His contribution becomes all the more impressive when i consider many personal challenges that he himself endured over the years.
I was quite impressed by kindness, commitment, determination, drive , respect and dedication. He has truly inspired me with his selflessness, great performance, persuasiveness, resourcefulness, persevering advised and innovative case manager.
It is my deepest honor and highest respect to nominate Mr. Paul Standish as the best Case Manager.
